Comments:
An unannounced renewal inspection was initiated on 4/15/2024. The inspector was on site from 9:30am until 12:00pm.

There were 8 children, ages 6 months to 4 years old under the supervision of one staff totaling 24 points. Children were observed during indoor free play and snack time. Four children's records, one provider record and one assistant record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Description: Based on record review, the provider failed to ensure Central Registry results were obtained by the end of 30th day of employment.

Evidence:
Assistant 1, date of hire approximately December 2023, had no record of a Central Registry on file during the inspection on 4/15/2024.

Evidence:
During the inspection on 4/15/2024, Assistant 1 was in a room in the back of the house from approximately 9:30am to 12:00pm. Assistant 1 only made contact one time during that time frame to say good morning to the children, then returned to the room. There were 8 children under 5 years old and the youngest was 6 months old.

During the visit on 4/15/2024, Child 2 (age 23 months) was in a highchair upon arrival. Provider stated the child was put in the chair due to biting twice that morning. To avoid another incident, the child was placed in the chair. The child remained in the chair from approximately 9:30am until 11:30am.
