Technical Assistance:
Discussed the importance of keeping the laundry room located on your upper level closed and locked when children are napping on that level.

The revised evacuation drill log will be emailed to use as a resource.

The model form for obtaining references will be emailed.

Comments:
An unannounced monitoring inspection was conducted today. A total of 10 children with 2 staff were onsite. The children had just completed transitioning inside from playing outside in the morning. Handwashing procedures were observed as they took turns washing hands and sitting down for lunch. Lunch today was chicken, potato fries and fresh grapes which were cut into small sections for the younger children. Discussed the need to ensure that children under the age of 3 are served food that is not considered a choking hazard. Walk thru of all 3 levels of the home was conducted. Observation of cribs, bathrooms, mats and the outside play area was made. The children finished eating lunch, washed hands and prepared for their afternoon nap. A total of 4 children's files, both staff and 1 household member file was reviewed. This inspection occurred between 11am and 1pm. Thank you for your cooperation today. Based on staff record review, one background check clearance was not received prior to hire.
Evidence:
Staff #2 was hired on 03/21/2024. The fingerprint background clearance was received on 03/26/2024.

Plan of Correction: I know fingerprint results are required to be obtained before they start.

Description: REPEAT
Based on child record review, the provider failed to complete an annual review.
Evidence:
1, Child #2's last review of emergency contacts was in November 2022.
2. Child #4's last review of emergency contacts was in December 2022.

Plan of Correction: I will review this with the parents.

Description: Based on review of the provider's emergency evacuation drill log, monthly drills were not practiced.
Evidence:
The last evacuation drill was conducted in February 2024.

Plan of Correction: We will have a fire drill before the end of April.

Based on staff record review, references were not obtained for one staff.
Evidence:
At least two references were not obtained for Staff #2.

Description: Based on review of child records, documentation of additional immunizations was not obtained for 1 child.
Evidence:
Child #4 is under the age of 2 years. The most recent documentation of immunizations were dated 05/16/2023.

Plan of Correction: I will make sure all parents provide updated immunizations to me.

Standard #: 8VAC20-800-240-A
Description: Based on observation of the outside play area, the area was not maintained in a safe condition.
Evidence:
1. The wooden swing had splintered wood at the base of the seat.
2. The bottom step of the ladder on the playhouse equipment was split, splintered with a rusty nail.

Plan of Correction: The step will be removed. The swing will be removed.

Standard #: 8VAC20-800-250-A
Description: Based on observation of the play equipment, a rope was within reach of children under the age of 5 years.
Evidence:
A climbing rope on the play equipment located in the backyard was no longer anchored and was accessible to the children in care.

Plan of Correction: The rope will be removed ASAP.
