Comments:
An unannounced monitoring inspection was conducted on 4/24/24 from 8:20 a.m. to 9:00 a.m. with the provider. There were five children in care, ranging in age from two to five-years-old, for a total of 8 points. Children were observed in an activity and getting ready for a field trip. Five child records and two staff records were reviewed. The provider has current certification in CPR and First Aid. three medications and authorization forms were reviewed, and the provider has current certification in Medication Administration Training (MAT) The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 8VAC20-800-710
Description: Based on review of medication, the provider did not ensure that medication administration requirements were met in that only those drugs that were dispensed from a pharmacy and maintained in the original, labeled container may be administered by the provider.

Evidence: The prescription emergency medication on site for Child #6 was not maintained in a labeled container indicating that the medication was dispensed from a pharmacy.

Plan of Correction: Parent provided labeled container for child.

Standard #: 8VAC20-800-50-A
Description: Based on review of the written attendance record and interview with the provider, the provider did not ensure all children were included on the written attendance record.

Evidence: There was no written record of attendance.

Plan of Correction: Updated the attendance sheet.

Standard #: 8VAC20-800-240-A
Description: Based on observation, the Provider did not ensure that areas and furnishings of the family day home, inside and outside were maintained in a safe condition.

Evidence: The carpet in the daycare area had a large wrinkle going down the center of the carpet and the rug under the table had curling edges posing a trip hazard.

Plan of Correction: Carpet is scheduled to be replaced and the rugs have been removed.
