Comments:
An unannounced renewal inspection was conducted on 5/1/24 from 10:20am to 11:40am with the provider, who was present with one assistant. There were seven children in care, ranging in age from 10-months-old to four-years-old, for a total of 23 points. Children were observed coming in from outside, playing with playdough, doing puzzles, dancing, doing stretches, having their diapers changed, and washing hands. Four child records, two caregiver records, one household member record, and two pet records were reviewed. The provider and assistant have current certification in CPR and First Aid. The first aid kit, flashlight, weather-band radio, and carbon monoxide detector were observed. The emergency drill log, attendance, and menu were also reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 5/7/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-800-60-B
Description: REPEAT VIOLATION Based on review of four child records, the provider did not obtain all of the required documentation for child records. Evidence: The record for Child C did not contain the work place name, address, and phone number for the parent, or the address of a second emergency contact if a parent cannot be reached.

Plan of Correction: Parent took care of this portion today.

Standard #: 8VAC20-800-90-C
Description: Based on review of four child records, the provider did not obtain documentation of additional immunizations for children every six months for children under the age of two years. Evidence: The records for Child A, Child B, Child C and Child D did not contain updated immunizations for children every six months until their 2nd birthday.

Plan of Correction: Parents are all notified and will provide proof by 5/13/24.

Standard #: 8VAC20-800-180-A
Description: Based on review of two caregiver records, the provider did not obtain for each caregiver a current tuberculosis (TB) screening every two years. Evidence: The most recent TB screening on record for Caregiver A was dated 4/13/22. The most recent TB screening on record for Caregiver B was dated 4/20/22.

Plan of Correction: Appointments re-scheduled for today.

Standard #: 8VAC20-800-460-C
Description: Based on review of two pet records, the provider did not ensure to maintain current rabies vaccination for dogs. Evidence: The pet records contained documentation that the rabies vaccination for both pets expired on 3/24/24.

Plan of Correction: Appointment made with the vet for 5/11/2024.
