An unannounced monitoring inspection was conducted on January 28, 2020 to review subsidy health and safety inspection requirements. Provider was present with three children for a point total of ten. Provider and three children records were reviewed.

Standard #: 22VAC40-665-190-B Description: Based on review, provider did not ensure caregivers undergo TB screenings at least every two years from the date of initial screening.
Evidence: The most recent TB test/screening for provider was dated February 3, 2017 which is more than two years old.Plan of Correction: Provider will go to the health department to have the test or screening.
Standard #: 22VAC40-665-230-H Description: Based on review, provider did not ensure caregivers complete at least sixteen hours of training annually.
Evidence: There was documentation of two hours of training completed in 2019. Provider stated no other trainings were completed in 2019.Plan of Correction: Provider will start working on annual training online.
