Comments:
An unannounced monitoring inspection was conducted today from 1:40 pm-2:40 pm. There were 23 children directly supervised by 3 staff. The physical plant, staff records, children' records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There was an abundant supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, staff-to-child ratios were in compliance. The school was clean and organized. Children were observed: playing legos, video games, and resting. Infants were observed doing tummy time and getting ready for a bottle. All violations from previous inspection had been corrected. The playground equipment and fencing had been updated and a new system was implemented to document annual record reviews. Standard #: 22VAC40-185-130-C
Description: Based upon review of children's records, required documentation was found to be incomplete or missing. Evidence: Child B's (start date 09/2014) record did not contain current immunization record.

Plan of Correction: Director will have parent provide a copy of the child's immunization record from the elementary school.

Standard #: 22VAC40-185-60-A
Description: Based upon review of children's records, required documentation was found to be incomplete or missing. Evidence: Child A's record is missing documentation of an annual file review. Last review on file 12/31/2012.

Plan of Correction: Director will have parent update file at pick up.

Standard #: 22VAC40-185-270-A
Description: Based upon observation , areas of the center were not maintained in a clean/safe/and or operable condition. Evidence: The hand rail going down the stairs to the back playground was loose on the left side of the stairs. The sinks in children's rest room on the lower level had rusted connectors that were accessible by the children.

Plan of Correction: The director will repair both the sinks and the hand rail.

Standard #: 22VAC40-185-450-A
Description: Based upon observation, children in care did not have top/bottom cover during nap as required. Evidence: Three children in the 2- 4 year old sleep area did not have a top and bottom cover as required. The children were observed resting directly on the nap mat with a blanket on top.

Plan of Correction: The teachers will remind parents to provide sheet and blanket for their child.
