Comments:
Conducted an unannounced monitoring study with the director, eleven staff, and 56 children. The ages of the children range from three months-old through five years-old. Three new staff records and five children's records were reviewed as well. There were no children on medication today. The children were eating lunch and napping during the inspection. Start time: 10:00am End time: 12:20pm

Description: Based on a direct observation of the licensing binder, the annual fire inspection was not conducted annually. Evidence: The most recent annual inspection was conducted on 02/10/2014, and was due on or by 2/10/2015.

Plan of Correction: The Director called the fire department during the inspection to check on a new inspection date.
