The licensing inspector initiated an unannounced inspection in the center on October 14, 2015 and completed in the center on October 30, 2015 in response to a self-reported incident received by the department on October 13, 2015 regarding the area of supervision. The investigation was conducted jointly with a local agency. Related documentation was reviewed and interviews were conducted with center staff and with other parties involved. Based on information obtained during the inspection the preponderance of evidence does support the allegation therefore the self-reported incident is noted as valid. The results of the inspection were reviewed and verified with the center director during the exit interview on October 30, 2015.
Standard #: 22VAC40-185-340-A Description: Based on interviews with staff, the facility failed to ensure care, protection and guidance of a child in care. Evidence: 1. On October 13, 2015, child #1, (age 21 months) was left alone on the playground at approximately 3:20 PM when two teachers and seven children left the playground and went into the center. 2. On October 13, 2015 center staff did not know the whereabouts of child #1 for at least 20 minutes (approximately 3:20 PM - 3:40 PM). 3. Center staff were unaware that child #1 was missing until the guardian arrived to pick up the child at approximately 3:40 P.M. 4. Staff # 1 found child #1 on the playground at 3:40 PM. Staff #1 was interviewed and confirmed that at approximately 3:40 PM child #1 was found standing on the playground against the fence and near the playground entrance. Staff #1 said when child #1 was found, the child was not crying or visibly upset. The weather during this incident according to www.weather.com was fair and 75 degrees Fahrenheit. 5. Staff #1 walked the licensing inspector through the route the children take to the playground. After exiting the classroom, they go about 3 feet and exit a metal door then walk approximately 286 feet to the playground. The playground is located behind a brick building and is surrounded by a 3 ? metal chain link fence. The center is located on the grounds of a public high school and the playground is directly behind the school and next to the sports fields. A key tag is required to enter and exit the center. 6. The inspector determined through interviews with center staff that the care, protection and guidance of child #1, was compromised when child #1 was left without supervision on the playground appropriately 20 minutes. Plan of Correction: The two staff responsible for leaving the child behind were terminated. Extra training for supervision and playground for all staff was conducted. Additional paperwork and procedures for frequent head counts were added.
Standard #: 22VAC40-185-340-F Description: Based on interviews, the facility did not ensure children under 10 years of age were within actual sight and sound supervision of staff. Evidence: 1) Child #1, age 21 months, was left alone without sight and sound supervision of staff on the outside playground on October 13, 2015 from approximately 3:20 PM - 3:40 PM. 2) The playground is located behind a building adjacent to the building which houses the center. 3) The center staff were not able to see or hear the child. Plan of Correction: The two staff responsible for leaving the child behind were terminated. Extra training for supervision and playground for all staff was conducted. Additional paperwork and procedures for frequent head counts were added.
