Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection November 23, 2015 from approximately 10:30am to 2:00pm. The following ratios were observed with a total of 45 children present during the inspection:
Infants - 6 children with 2 staff
Toddlers - 9 children with 2 staff
Twos - 11 children with 2 staff
Threes (Preschool) ? 9 children with 2 staff
Fours & Fives (Pre-K) - 11 children with 2 staff
School Age children were not present during the inspection.

During the inspection, the Infants were observed having their diapers changed, eating lunch, being rocked and sleeping in their cribs. The Toddlers were observed playing outside, eating their lunch and listening to music. Twos classroom was observed playing outside, washing their hands, having their diapers changed and taking bathroom breaks, looking at books and eating lunch. The Threes classroom was observed having a book about recycling read to them, working puzzles, building with legos, and playing with table top toys. The Pre-K classroom was observed coloring pictures of Thanksgiving turkeys and talking about why they are thankful. Lunch consisted chicken and rice, potato wedges, apricots, and milk. All areas of the center including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children.
The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, daily attendance, infant feeding schedules, parent teacher conferences, children progress reports, menu, daily sheets, parent agreements, and staff training information. Medication is administered and medications and medication authorizations were inspected.

Standard #: 22VAC40-185-130-A
Description: Based on review of five children's records, one record did not contain documentation of immunizations prior to the child attending the facility.

During the inspection, the licensing inspector observed the record for Child #3 (enrollment date 10/05/15) contained documentation of immunization records dated 11/10/15.

Plan of Correction: Per director - this family had recently moved to the area from another state and it took a bit of time to locate a pediatrician.

Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, one record did not contain a physical examination by or under the direction of a physician.

During the inspection, the licensing inspector observed the record for Child #3 (enrollment date 10/05/15) did not contain documentation of a physical exam. Each child's record is required to contain a physical exam prior to or within 30 days of attending the center.

Plan of Correction: Per director - the parent of Child #3 was called during the inspection. The parent thought the child's physical was faxed with the immunization documentation. The parent will notify the pediatrician and request the office to send the physical exam again.
