Discussed with center administration: Alterations to medication forms, maintaining proper staff documentation on site for MAT certifications.
An unannounced monitoring inspection was conducted today. There are 4 staff with 63 children after school. The playground was not inspected today due to inclement weather. One staff was able to show current MAT certification, after the main office was contacted. Please place documentation at the facility of all current MAT certifications. 4 children's files were reviewed. All were complete. Staff files are reviewed at another time at the main office. An emergency escape drill should be conducted before the center closes today. First aid supplies, menu, emergency supplies and the emergency preparedness plan are all in compliance. Standard #: 22VAC40-185-510-C Description: Based upon a review of medications, one manufacturer's instructions were not met. Evidence: One medication, present at the center is expired. (October 2015). (Child 3) Plan of Correction: The parent will be contacted and asked to bring in current medication.
Standard #: 22VAC40-185-510-G Description: Based upon a review of medications, one authorization is not acceptable. Evidence: One authorization for administration of medication authorizes Loudoun County Public Schools (LCPS) to give the medication, not Loudoun County Parks and Rec. Plan of Correction: The parent will be given the correct form for completion.
