22VAC40-185 STAFFING AND SUPERVISION.
The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 11/12/15. The investigation was first initiated on 11/12/15 by a telephone conference with the center director and District Director. Interviews were conducted on 11/17/15 with management and staff relating to the allegations of a hazardous item being accessible to children . The classroom was observed and management provided the emails that had been shared between parents and management. The information gathered during this investigation supports the allegations, so the complaint is determined to be valid. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 calendar days of receipt. Standard #: 22VAC40-185-280-H Complaint related: Yes Description: Based on staff interviews, licensee failed to ensure that harmful agents are stored in areas that are not accessible to children. Evidence: On 11/9/15 a box cutter was left out in the K- stem classroom and accessible to children. 1. Staff #1 was interviewed and said that she was using the box cutter in the morning to cut up boxes. She got distracted, putting the box cutter down. 2. Staff #2 was interviewed and said "a boy (child #3) came to me holding a box cutter and said, look what I have". Staff #2 placed the box cutter in the teachers desk. Staff remembers child # 2 standing with child #3 by the teachers desk. Plan of Correction: The harmful agents have been removed from the K-stem classroom and placed in a locked storage in the director's office. Teachers can check items out when needed and return immediately to locked storage. Staff have participated in safety and accountability retraining.
