Comments:
An unannounced monitoring inspection was conducted today. There are 2 staff and 26 children present after school. There are currently no medications at the center. Emergency supplies, Emergency escape drills, first aid supplies, menu and emergency preparedness plan are all in compliance. 2 staff files were reviewed for MAT certification and 1st and & CPR certifications. Staff files are reviewed at the main office at another time. 4 children's files were reviewed. See violation notice for missing information. The playground was not in use today due to inclement weather. If you have any questions regarding this inspection, please contact me at 703-479-4678. Description: Based upon a review of files, one child's file is incomplete. Evidence: Child #1 does not have documetnation of the Father's information.

Plan of Correction: I will ask the parent to provider the information.
