Comments:
An unannounced monitoring inspection was conducted from 9:20 a.m. - 1:15 p.m. There were thirty-eight children in care with six teachers working directly with them. There were other school personnel available to assist as needed. Two children's records were reviewed. Reviewed were the records for three teachers. The children were observed making gingerbread cookies to decorate, coloring and drawing, making pomanders, playing pretend in their yellow gnome capes, working with the mineral kingdom using shells, rocks and little gnome characters set in a wooden vessel, washing hands, setting the table with cloth napkins and during snack. Description: Based on a review of inspections the facility failed to have an annual health department inspection available for review. Evidence: 1. On 12/02/2015 the licensing inspector reviewed the most recent health inspection which had been conducted 09/18/2014. 2. On 12/02/2015 staff #1 confirmed the most recent health inspection had been conducted 09/18/2014.

Plan of Correction: The request for the annual health inspection was made in August. There have been follow-up telephone calls and emails in an effort to meet this requirement. I was able to speak with the health official today and this inspection will take place tomorrow. I will send a copy of our inspection report to the licensing inspector for the record.
