Comments:
An unannounced monitoring inspection was conducted today. Observed 35 children directly supervised by 4 staff members. Ratios and supervision were in compliance. A sample of 2 children's records, and 4 staff emergency contact sheets were reviewed. The physical plant, medications, emergency supplies and procedures were also reviewed. The children were engaged in various activities, to include: free play, hand washing, basketball, coloring, homework, and snack time. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-270-A
Description: Based upon observations made, an equipment of the center was not kept in a safe condition. Evidence: The pretend play sink was missing the faucet piece; which left a hole exposed, creating a possible finger entrapment hazard.

Standard #: 22VAC40-185-510-C
Description: Based upon documentation and medication reviewed, a child's long term medication did not have a written authorization from the child's physician. Evidence: One medication (EpiPen Jr.) did not have a written authorization from the child's physician (Child A).

Standard #: 22VAC40-185-560-F
Description: Based upon observations made and interview, children were not allowed second helpings of food listed in the menu for snack. Evidence: 1) The menu listed the following items for snack: tomato basil crackers with cheese, yogurt cup, and a fruit cup. 2) There was a limited number of tomato basil crackers with cheese available for the children, the quantity was not enough to offer these items as a choice to all the thirty five children during today's inspection. 3) Staff interviews confirmed that the tomato basil crackers with cheese listed on the menu were left over snacks from a previous day. 4) Children were observed walking from table to table and looking in the "share bowl" for cheese. When the children did not find cheese, they would placed their yogurt and or fruit cup inside the "share bowl" and sit down. 5) One of the children who was able to get cheese asked for more cheese, a second helping, and Staff A told him no as they did not have anymore cheese.
