Comments:
On 10/05/2015, an unannounced inspection with local agency personnel was conducted in response to a self reported incident concerning supervision of a child. Interviews were conducted, records and video tape were reviewed. Based on a preponderance of the evidence, a violation was found and a Violation Notice was issued. Standard #: 22VAC40-185-340-A
Description: Based on interviews, review of records and video tape, staff failed to ensure the care and protection of a child with special needs. Findings: It was found that on 9/28/2015, a four year old child with special needs was able to exit the building and go outside alone. Staff were unaware the child had left the building. The child was out of sight and sound supervision for approximately 20 minutes before staff found him outside near the playground area.
