Technical Assistance:
The inspector discussed with the management staff standard 22 VAC 40 80 150, of the General Procedures and Information for Licensure, regarding approval of buildings and functional design features.

Comments:
An unannounced inspection to renew the license was conducted on December 11, 2015 from 2:41 p.m. to 5:25 p.m. During the inspection there were 47 children, ages 2 years to 11 years old, with 8 staff present. Records were reviewed for 4 children and 2 staff. There were 3 medication reviews. Children were observed while resting, arriving from school, eating snacks, engaging in outdoor play, crafts, block play, card games and departure with parents. Areas of noncompliance were found in the administration and physical plant. The violations are identified on the violation notice and were discussed with the director during the exit interview

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of 4 records, the facility did not ensure that children's records had documentation of previous child day care and schools attended by the child. Evidence: 1. The record of child #3 did not list an answer regarding previous child care or school attendance. 2. Management confirmed that the information was not listed.

Plan of Correction: The child's file has been pulled to catch te parent to complete application form. Previous child care will be added to children's file checklist to ensure completion.

Standard #: 22VAC40-185-60-A-8
Description: Based on a review of 3 records, the facility did not ensure that documentation was of chronic physical problems and pertinent developmental information and any special accommodations needed was on file. Evidence: 1. The record of child #4 did not have have answer regarding chronic conditions. 2. Managment confirmed that the information was not in the record.

Plan of Correction: The child's file has been pulled to catch the parent to complete application form. Chronic physical problems[pertinent developmental information/special accommodations will be added to children's file checklist to ensure completion.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. A small, clear spray bottle, labeled bleach water, was stored in an unlocked cabinet on a shelf that was 53 inches from the floor in a preschool classroom. 2. Management confirmed this.

Plan of Correction: A child safety device will be installed to the cabinet int he Red room that holds the bleach water bottle.
