An unannounced monitoring inspection was conducted today at 7:50am. There were 7 school-agers directly supervised by 2 staff. The physical plant, 5 limited staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed arriving to care, and sitting and playing small games with the staff and other children. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Evidence: 1) Reviewed 5 children's records and found that CH1 did not have documentation of and address for the second emergency contact. 2) Reviewed 5 children's records and found that CH1 did not have documentation of the emergency contact information being annually reviewed by the parents.
