TA was provided on when a MAT trained person needed to be at the center.
The licensing inspector conducted an unannounced complaint inspection on 11/ 03/15 and 11/05/15 in response to a complaint received in the licensing office on 11/2/2015. Interviews were conduced with staff relating to the allegation of inappropriate behavioral guidance. The preponderance of the evidence gathered during the investigation does not support the allegation so the complaint is determined to be "not valid". However, during the investigation there was a unreported found violation. Areas of non-compliance are listed on the violation notice. Please contact your inspector with any questions.
Standard #: 22VAC40-185-410-7 Complaint related: No Description: Based on interviews, the facility staff punished a child for toileting accidents. Evidence: 1. The inspector interviewed three staff members relating to toileting accidents. Staff #1 reported that child #1 at times has accidents of pooping in her underwear and that her guardian instructed staff to have the child clean up after herself by washing out her underwear in the sink. Staff #2 reported helping child #1 clean her underwear after having an accident. Plan of Correction: Center staff will not have children clean their underwear after an accident. Center director will instruct all staff on how to handle accidents. Director will communicate with family about the child's health needs and how they can be addressed.
