22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today, December 16, 2015 to determine the center's compliance with licensing standards. Most but not all standards were reviewed during today's inspection. The inspector arrived at 10:00 am and concluded the inspection at 1:00 pm. Upon the inspectors arrival, children were observed in a variety of activities. Children in lower elementary were working on grammar boxes, journals entries, geometry and time. The upper elementary children were working on academics with science and creative writing and math. Preschool and kindergarten children on the second floor were observed reading, participating in activities with practicing skills to determine left and right, sorting and cutting. Children also played with magnetic blocks and washed toys at the water table. Snack was also observed. Children provide their own meals. Staff interacted in a positive manner with children as they circulated around the room. The facility has age and stage appropriate toys and materials. The facility had the following posted: License, last inspection, emergency numbers, evacuation maps/procedures, daily schedule and various parent information. The first aid kit and non-medical emergency supplies were inspected and found complete on both floors of the facility. The playground has had additional mulch added. The following ratios (staff to children ) were observed in each classroom: Lower elementary: (6-8 years): 27 children with 2 staff. Upper elementary: (9-12 years) 14 children with 1 staff. Kindergarten: (5-6 years) 15 children with 2 staff. Preschool: (3-6 years) 10 children with 2 staff while napping. The last evacuation drill was conducted on 11/16/2015. Two shelter-in-place drills were documented on 06/09/2015 and 11/16/2015. The Emergency Preparedness Plan was reviewed on May 9, 2015. The facility has current liability Insurance. Health Inspection: 02/02/2015 Fire Inspection: 1/28/2015 There were no medications on site. Multiple staff have current EMAT certification. Staff have current CPR & First aide training along with Daily Health Observation training. The violations from the previous inspection on May 13, 2015 were checked for corrections. Standard #: 22VAC40-185-160-C
Description: Based on review of staff records, the facility did not obtain a follow-up tuberculosis screening at least every two years from the date of the first initial screening for each staff. Evidence: The most recent tuberculosis screening for staff #1 was dated 09/18/2013, exceeding the two year allowance.

Plan of Correction: Per the Director: We will have the staff obtain an up-dated tuberculosis screening right away.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's records, the facility did not obtain all required information for each child enrolled. Evidence: 1. The record of child #1, enrolled 07/09/2012, did not have the address listed for one emergency contact as required. 2. The record of child #4, enrolled 03/08/2008, did not have the addresses for the two required emergency contacts.

Plan of Correction: Per the Director: I will contact the parent today and have them update the record with the addresses for the emergency contacts.
