Technical Assistance:
Repair request has been submitted for fence.

An unannounced monitoring inspection was conducted today from 11:00 am-12:30 pm. There were 48 children directly supervised by 11 staff. The physical plant, staff records, children' records, children's medications records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There was an abundant supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, staff-to-child ratios were in compliance. The center was clean and organized. The children were observed participating in the following activities: dancing with scarves, playing peek a boo, building with blocks, having a play-dough birthday party and washing hands at required times. Description: Based upon record review, required documentation was incomplete or missing. Evidence: Child A's record was last updated in 2014. Child B's record was last updated 8/2014.

Plan of Correction: The center will review dates on re-registration forms to have documentation of this standard.

Standard #: 22VAC40-185-70-A
Description: Based upon record review, required documentation was incomplete or missing. Evidence: The sworn statement for staff 1 expired on 10/5/2015 and needs to be updated.

Plan of Correction: The employee will sign a new form today.

Standard #: 22VAC40-185-270-A
Description: Based upon observation, areas and equipment in the center are in need or repair. Evidence: In the 2-3's classroom the cabinet next to the door has exposed edges, and the top of the blue and yellow shelves ate chipped and have sharp edges. In the pre-school classroom the block shelf has plastic trim that is detached and has sharp edges. The after school room stage area floor is in need of repair. Older Playground: siding with sharp edges near the door to enter and exit the building. Paint is peeling on the stairs to the play structure. Blue trike with rusted wheels and seat. Younger playground: Paint on the wagon is peeling and the pink push car has a broken strap. The surface that meets the sidewalk on both playgrounds is not deep enough and poses a tripping hazard.

Plan of Correction: An ongoing work order has been submitted.

Description: Based upon observation, infant bottles were not labeled as required. Evidence: There were seven infant formula bottles in the refrigerator that did not have the date as required.

Plan of Correction: The teachers will remind parents and check at drop off.
