An unannounced monitoring inspection was conducted on 1/7/16 from 3:00pm - 4:45pm. The licensing Inspector arrived, and observed the intake of 15 school age children and two staff. A tour of the facility was conducted and children were observed doing homework, playing games, and eating snack. Records were reviewed for five children and two staff. Medication was reviewed along with the emergency preparedness plan, transportation procedures, and emergency supplies. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-185-290-3 Description: Based on observation, it was determined that the facility did not ensure that all electrical outlets that are not in use shall have protective covers that are of a size that cannot be swallowed by children. Evidence: 1. There was an uncovered electrical outlet located above the sink and below the cabinets in the left classroom. 2. There were 3 uncovered electrical outlets on a surge protector located under the window on the back wall in the left classroom. 3. Staff #2 (Prgram Director) confirmed that the unused electrical outlets did not have protective covers. Plan of Correction: All unused electrical outlets will have protective covers. Staff will be trained to visually inspect electrical outlets to make sure they have protective covers when not in use.
