An unannounced monitor inspection was conducted on January 6, 2016. Upon arrival, there were twenty-one (21) children ages 6 months ? 4 years old, the Owner, Center Director & six (6) staff members present. Three (3) additional school age children arrived during the inspection in the afternoon. Attendance in each group was as follows: Infant Classroom There were five (5) children ages 6-15 months old & two (2) staff members present. Toddler Classroom There were four (4) children ages 16 months-23 months & one (1) staff member present. 2 Year Old Classroom There were six (6) 2 year old children & two (2) staff members present. 3-4 Year Old Classroom There were two (2) 3-4 year old children & one (1) staff member present. School Age Three (3) school age children arrived during the inspection under the supervision of one (1) staff member. Children/staff/medication records were reviewed. Center does not transport children & does not administer medication. Children were observed during arrival departure procedures, active/quiet activities, self-initiated and directed activities; bath room/diapering/hand washing procedures, meals/snacks served & nap time. LI Anderson concluded inspection at 5:05 pm. Risk Assessment & areas of non-compliance found during today?s inspection were reviewed with the Owner on 1-06-2016.
Standard #: 22VAC40-185-270-A Description: Based on inspection, the Licensee failed to keep areas and equipment maintained in a clean, sage and operable condition. EVIDENCE: In the 3 year old pre-school classroom located on the 2nd floor, LI Anderson observed cords on horizontal blinds hanging low enough for the children in care to grab & long enough for it to be a strangulation hazard. Plan of Correction: Center owner will purchase hooks to keep cords out of reach of the children.
Standard #: 22VAC40-185-370-3 Description: Based on discussion with staff, the Licensee failed to ensure infant daily activities include outdoor time if weather and air quality allow based upon the Air Quality Color Code Chart. EVIDENCE: Daily outdoor time is not provided for infants. Plan of Correction: Children will have daily outdoor time if weather permits.
Standard #: 22VAC40-185-570-C Description: Based on inspection & discussion with the Staff, the Licensee failed to keep a record of each child on formula to include brand of formula & feeding schedule. EVIDENCE: Infant records for five (5) children did not include the feeding schedule for each infant. Plan of Correction: Director will ask parents to document feeding schedule.
Standard #: 22VAC40-185-570-I Description: Based on inspection & discussion with staff, the Licensee failed to have one-day's emergency supply of commercial formulas appropriate for the children in care. EVIDENCE: Emergency infant formula is not available for infants in care. Plan of Correction: Director will purchase center provided formula for emergencies. Parents will be notified of the brand and given the option to bring their own brand of emergency formula.
