Comments:
the center had 45 children present with 9 staff at the beginning of this monitoring visit, 21 children left for school after the first half hour of the visit. The children were observed playing with manipulative, reading books, working on computers, using the listening center, playing with blocks and trains and play dough. Lunch, fish sticks, peas and pears with milk to drink, was also observed as well as nap time. The center has lots of supplies for the children to explore and lots of interaction between the teacher and children were observed in all the rooms. Standard #: 22VAC40-185-130-A
Description: The center failed to get the immunization records for 2 children before they started at the center. Evidence: Based on the review of 8 children's files , 2 children, child G and H are missing immunization records, the children started at the center on 3/24/15 and 1/11/16.

Plan of Correction: Already got child G's during the visit , will get the information from the other child this evening.

Standard #: 22VAC40-185-140-A
Description: The center failed to get a physical for 1 child out of 8 children's files reviewed. Evidence: Based on the review of 8 children's files the center did not get a physical after the first 30 days. The child, child G, started at the center in March of 2015.

Plan of Correction: I have asked the parent for the physical on the correct form.

Standard #: 22VAC40-185-60-A
Description: The center failed to get some of the information needed for 3 out of 8 children's files reviewed. Evidence: Based on the review of 8 children's files , child B,F and G do not have the address for the second emergency contact listed in the file.

Plan of Correction: We will ask for the information.

Standard #: 22VAC40-185-280-B
Description: The center failed to keep cleaning items locked up. Evidence: Based on the physical plant review, the inspector found cleaners in the twos room, cleaners in an unlocked cabinet.

Plan of Correction: fixed during the visit.

Standard #: 22VAC40-185-280-D
Description: The center failed to keep cleaning agent separated from food. Evidence: Based on the physical plant review the infant room has a storage cabinet that has cleaners and formula on the same shelf.

Plan of Correction: We will make one shelf a cleaning shelf on the bottom of the cabinet.
