Comments:
An unannounced renewal inspection was conducted on February 11, 2016. There were 18 children present with two staff members. The children were observed coloring, playing with board games, having a discussion about current events and playing with other tabletop activities. The licensing inspector reviewed the physical plant, supervision, required postings, children's records, emergency evacuation and shelter-in-place documentation, and nutrition. The posted menu indicated that today's snack consisted of applesauce and graham crackers. There are no medications present or being administered in the program. Three children's records were reviewed during the inspection.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review in two of three children's records reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: 1. During the inspection conducted on February 11, 2016, the record for Child #1 did not include the documentation of an address for one of two persons to contact in the event of an emergency where the parent cannot be reached. 2. The record for Child #2 did not include documentation of employment information and work phone numbers for both parents, the names of previous child day care and schools attended by the child and a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.

Plan of Correction: Called parent and added address for Child #1 Parent completed form that indicates work phone numbers for both parents, the name of previous child day care and schools attended by the child and the statement that the parent will inform the center within 24 hours about any communicable diseases, that should be reported to the State Board of Health.

Description: Based on review, the center did not ensure that a written record of injury was documented with all required information. Evidence: During the inspection conducted on February 11, 2016, the injury report reviewed did not include documentation of how the parent was notified of the injury and the time the parents were notified of the injury.

Plan of Correction: Included documentation of how the parent was notified of the injury and the time the parents were notified of the injury.
