Comments:
The licensing inspector conducted an unannounced monitoring inspection February 25, 2016 from approximately 1:45pm to 4:15pm. The following ratios were observed with a total of 61 children present during the inspection:
Infants - 5 children with 2 staff
Toddlers - 5 children with 1 staff
Twos - 12 children with 2 staff
Threes (Preschool) ? 10 children with 2 staff
Fours & Fives (JK) - 14 children with 2 staff
School Age - 15 children with 1 staff

During the inspection, the Infants were eating snack and being held. The Toddlers were observed eating snack and having the book ?My Treasury of Nursery Rhymes" read to them. Later the children went outside to play. The Twos were observed playing outside and later playing with toys in their classroom including building with legos, playing with trains, and playing in the kitchen area. The Preschool children were observed playing outside, participating in circle time where the staff reviewed dental health with the children. The children were observed looking at shark teeth as part of dental health week. The JK children were observed playing outside and then later participating in center time in their classroom. The children were observed building with magnetic blocks, working puzzles, coloring rainbows, and playing in the housekeeping area. The School Age children were observed arriving to the center from the public school system, playing board games and completing homework. Afternoon snack consisted of string cheese, crackers and water. All areas of the center including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children.

The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, daily attendance, daily sheets, parent agreements, and staff training information. Medication is administered and medication authorization and medications were checked.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #1 (enrollment date 02/03/2016) did not contain the physical address for one of the two emergency contacts listed in the child's record.

Plan of Correction: Per director - I will speak to the family and obtain the physical address for the other emergency contact. We will add the information to the child's record.
