22VAC40-191 Background Checks (22VAC40-191)

An unannounced renewal inspection was conducted today. There were 22 (infants-5yrs.) directly supervised by 5 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed arriving to the facility and participating in circle time. There was an abundant supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) Observed hand sanitizer accessible to children on the halfway separating the school-age room from the main hallway. 2) Observed in the school-age room, a storage room with spray paint, bleach water and disinfectant spray unlocked.

Plan of Correction: All cleaners and hazardous substances were removed from access to school-age children and cabinets containing these substances will be locked.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the facility failed to ensure that the diapering surface is nonabsorbent. Evidence: Observed that the diaper changing mat in the Infants Room had rips causing it to be absorbent.

Plan of Correction: Diaper pad was replaced with a new pad.

Standard #: 22VAC40-185-550-M
Description: Based on review, the facility failed to ensure that children's injury records were maintained with the required information. Evidence: Reviewed 5 injury report records and found that 1 did not have documentation of any future action to prevent recurrence, and 2 did not have documentation of the time/date parents were notified of the incident.

Plan of Correction: A Staff training was conducted on the proper method of completing injury reports.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the facility failed to ensure that food that is brought from home is labeled in a way that identified the owner. Evidence: Observed a lunch bag and food in the fridge that was not labeled with the date or the name of the child.

Plan of Correction: Parents will be told and staff will ensure that all food is labeled correctly before it is left at the center.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the facility failed to ensure that the findings from the most recent inspection of the facility was posted. Evidence. Observed that the most recent findings posted was dated 9/11/14. The facility has had 2 inspections since then, one on 3/31/15 and a second on 7/20/15.

Plan of Correction: All inspections notices will be posted on the parent information board to ensure parent access.
