Comments:
An unannounced monitoring inspection was conducted on March 2, 2016 from approximately 9:45 am to 1:30 pm. Seven staff were present with forty-eight children. Children were observed during story time, art, circle time, outside play, center time and lunch. Five children?s records and three staff records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, hand washing procedures, nutrition, special feedings, injury report documentation, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Violations were reviewed with and verified by the Program Director on this date.

Standard #: 22VAC40-185-140-A
Description: Based on review, facility did not ensure 1 of 5 children's records have a physical within 30 days of beginning attendance. Evidence: On March 2, 2016, the record for child #5 (start date August 31, 2015)

Plan of Correction: Will notify parent of most up to date physical with correct signage or stampage.

Standard #: 22VAC40-185-150-B
Description: Based on review, facility did not ensure 1 of 5 children's records has an acceptable for of immunization record to include a signature or stamp of the physician, physician's designee or local health department official. Evidence: On March 2, 2016, the record for child #5 (start date August 31, 2015) has an immunization record that is not signed or stamped by the physician, physician's designee or health department official.

Plan of Correction: Will notify parent of most up to date shot record with correct signage or stampage.

Standard #: 22VAC40-185-160-A
Description: Based on review, facility did not ensure 1 of 3 staff records has documentation of a negative tuberculosis test or screening to be completed no more than 12 months prior to beginning employment. Evidence: On March 2, 2016, the record for staff #2 (start date July 31, 2015) has a TB test completed June 30, 2014 which is more than 12 months prior to beginning employment.

Plan of Correction: Staff will be notified today to get a most current TB test.

Standard #: 22VAC40-185-60-A
Description: Based on review, facility did not ensure 3 of 5 children's records have documentation of required information. Evidence: On March 2, 2016, the following was found: 1. The record for child #3 (start date February 8, 2016) does not have documentation of 2nd emergency contact address and previous programs attended. 2. The record for child #4 (start date February 16, 2016) does not have documentation of father's employment phone number and viewing proof of child's identity/age. 3. The record for child #5 (start date August 31, 2015) does not have documentation of previous programs attended and parent agreement to notify facility within 24 hours of household communicable diseases.

Plan of Correction: Program Director will speak with parents about missing information.

Standard #: 22VAC40-185-320-B
Description: Based on observation, facility did not ensure restrooms used by children have the required supplies. Evidence: On March 2, 2016, the following was observed: 1. There was no toilet paper and paper towels in the school age bathroom. 2. There were no paper towels in the 2's class bathroom. 3. There were no paper towels at the sink in the 4's class.

Plan of Correction: Food order with paper towels and toilet paper arrived during visit and will be stocked today.

Description: Based on observation, facility did not ensure there are disposable towels at the sink for diapering areas. Evidence: On March 2, 2016, there were not paper towels available at the sink by the diapering area in the infant room.

Plan of Correction: Food order with paper towels and toilet paper arrived during visit and will be stocked today.

Standard #: 22VAC40-185-550-D
Description: Based on review, facility did not ensure evacuation drills are conducted monthly. Evidence: On March 2, 2016, there was not documentation of a completed emergency evacuation drill in July 2015. Documentation was verified by Program Director.

Plan of Correction: Monthly drills will be done and documented.

Standard #: 22VAC40-80-120-E-2
Description: Based on review, facility did not ensure the results of the most recent inspection are posted in a area visible to the public. Evidence: On March 2, 2016, the inspection notices from the most recent inspection conducted in July 2015 were not posted.

Plan of Correction: Violation notice will be posted for public view today.
