22VAC40-191 Background Checks (22VAC40-191)
Today an unannounced monitoring visit was conducted beginning at 10:50am. There were 102 children present today with 18 teachers and the Director. Children were observed playing outside on the playground, listening to teachers read stories, playing with blocks, cleaning up, and preparing for lunch. Four medications and authorizations were reviewed today in addition to five children's files and 7 staff records. This inspection, including the exit interview, ended at 1:40pm. Standard #: 22VAC40-185-160-C Description: Based on record review, 4 staff records (#2,3,4 and 7) do not have updated TB test results dated within 2 years of the most recent results. Evidence: TB tests are last dated: 9/2013 for staff #2, 3/2013 for staff #3, 11/2013 for staff #4, and 10/2013 for staff #7. Plan of Correction: We will send all staff to get these renewed immediately. We will renew TB tests every two years.
Standard #: 22VAC40-185-40-J Description: Based on discussion, there is no documentation that injury prevention plans are reviewed annually and updated as needed. Plan of Correction: We review our accident reports and make changes accordingly. We will begin documenting our review of injury prevention plans annually and make changes if needed.
Standard #: 22VAC40-185-70-A Description: Based on record review, two staff records (#1 and 6) are missing documentation that background checks were submitted and returned with results within 3 years of the most recent results. Evidence: Staff #1 does not have a sworn statement that has been updated since 1/2012. Staff #6 does not have updated search results of a criminal history background or search of the central registry. The last dated results for both of these background checks is 2/2012. Plan of Correction: We will submit these background checks immediately and renew them every three years before the current ones expire.
Standard #: 22VAC40-185-260-A Description: Based on discussion, a current copy of an annual fire inspection cannot be located. Plan of Correction: We did have a fire inspection in October, 2015. During the inspection a request for a copy of the fire inspection from October, 2015 was made to the county. I will send this to you as soon as it is received.
Standard #: 22VAC40-185-290-3 Description: Based on direct observation, an electrical outlet was missing a protective cover by the sink in the intermediate classroom. Plan of Correction: We will place an outlet cover in the outlet. We will make sure outlets have protective covers placed in them after they are finished being used.
