An unannounced monitoring inspection was conducted on March 24, 2016. There were 46 children present with six staff members and a volunteer in three classrooms. The children were observed playing in centers, watching children's programming, and participating in music and movement. Two medications were reviewed during this inspection. Three children's records were reviewed during the inspection.
Standard #: 22VAC40-185-60-A Description: Based on record review in of children's records reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: 1. During the inspection conducted on March 24, 2016, the record for Child #1 did not include documentation of one parent's work phone number and place of employment, one parent's home address, complete addresses for two persons to contact in the event of an emergency where the parents cannot be reached and the name of any additional programs or schools that the child is concurrently attending and the grade or class level. 2. The record for Child #2 did not include documentation of any previous child day care and schools attended by the child and the name of any additional programs or schools that the child is concurrently attending and the grade or class level. 3. The record for Child #3 the name of any additional programs or schools that the child is concurrently attending and the grade or class level, one parent's place of employment and one parent's home address. Plan of Correction: We will update a new registration form for each student's file and have them completely filled out by parents.
Standard #: 22VAC40-185-260-A Description: Based on observation and inspection of the facility, the licensee did not ensure that the center provided to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on March 24, 2016, there was not documentation of an annual fire inspection report available to review for the facility. Plan of Correction: We will try to obtain the Fire Marshall inspection from the assistant principal. If necessary, contact the Fire Marshall's office directly.
Standard #: 22VAC40-185-270-A Description: Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: During the inspection conducted on March 24, 2016, on the wooden ramp leading to the entrance of the facility, the licensing inspector observed a picket that had become detached from the rail on one end exposing a rusted nail underneath. There was also an exposed rusted nail jutting from the rail. Plan of Correction: We will send a maintenance request to the Main Office.
