An unannounced monitoring inspection was condcuted on March 23 2016 between 12:10pm and 1:30pm. There were 21 children in the direct care of 5 staff members. Children were observed reading books, washing hands, eating lunch (whole wheat tortilla, chicken, cheese, green beans), and playing with blocks. A sampling of staff and children's records, the physical space, supervision, evacuation drills, fire and health inspections, and care of children were observed and inspected. Standard #: 22VAC40-185-290-3 Description: Based on observation, an electrical outlet was uncapped. Evidence: An outlet on the lower level of the center did not have a protective covering. Plan of Correction: We will do right away.
Standard #: 22VAC40-185-500-B Description: Based on observation, a diaper pad on the lower level of the center had a 1 inch rip in the outer layer making the diaper pad absorbent. Plan of Correction: We will replace the diaper pad.
