Today we discussed use of high chairs and program director qualifications.
An unannounced monitoring inspection was conducted today from 1:35pm through 2:45pm. There were six children present ranging in age from 19 months through four years. There were three staff present at the onset of the inspection; 2 teachers and 1 office assistant. Children were observed in teacher and self-directed activities including nap, transitions, rest room routines and snack. Staff report that the infant room nor the second floor of the center are in use at this time.
Standard #: 22VAC40-185-160-A Description: Based on record review and interview the center failed to ensure that each staff submit documentation of a negative tuberculosis screening within 12 months prior to or 21 days after employment. Evidence: 1 - The record for staff 3 contains a documented date of hire as 2/28/16. However, the tuberculosis test on file is dated 9/8/14. The documentation is greater than 12 months prior to employment. 2 - Staff 1 confirmed that the information is outdated. Plan of Correction: Center management will ensure that tuberculosis screening is dated within the allowable time frames. Current staff with dated results outside the allowed time will obtain updated information.
Standard #: 22VAC40-185-60-A Description: Based on record review and interview the center failed to ensure that each child's record contained all required information. Evidence: 1 - The records for children 1, 2, and 3 do not contain two complete emergency contacts as required. a. child 1 has a documented first date of attendance as 2/29/16 b. child 2 has a documented first date of attendance as 2/8/ 16 c. child 3 has a documented first date of attendance as 2/27/15 2 - Staff 1 and 2 confirmed the information was not present in the children's records Plan of Correction: Center management will create and implement methods for ensuring that all required is obtained and documented in each child's record.
Standard #: 22VAC40-185-200-A Description: Based on record review and interview the center failed to ensure that a qualified program director or a qualified back-up program director who meets one of the director qualifications who is regularly on site at least 50% of the center's hours of operation. Evidence: 1 - Staff 1 and 2 stated that the program director of record was not present and had been absent from the building since mid-January. 2 - Staff 1 stated that staff 4 had been designated as the back-up program director. 3 - The record for staff 4 does not contain documentation that the staff meets one of the program director qualifications. 4 - Staff 1 confirmed that staff 4 does not meet one of the program director qualifications. Plan of Correction: The center owners will ensure that a qualified program director is present during 50% of center's hours of operation.
