Comments:
An unannounced inspection was conducted today in response to a self reported incident. Based upon multiple interviews, the preponderance of evidence does not support the allegation. There is no violation associated with the report. Additional violation was found during the course of the investigation. Standard #: 22VAC40-185-60-A
Description: Based upon a review of files, one child's file is incomplete. Evidence: Child 1's file does not contain documentation of having viewed a Proof of Birth document. Date of enrollment: 10/27/2014.

Plan of Correction: I will e-mail the parents and ask them to bring the original document.
