Technical Assistance:
The tire toys beneath the dome climber must be moved away from the fall zone area prior to children using the playground.

Comments:
An unannounced monitoring inspection was conducted today. There were 40 children in care, with a total of 5 staff and 5 volunteer co-op parents assigned to their direct supervision. The children were observed engaging in a large variety of large and small group activities, including: attending music class, playing with the bead table, building with small and large blocks, playing with the train set, reading books, creating art projects, and eating snack. This inspection was completed between approximately 9:45 am and 11:00 am. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #4 does not include the father's work phone number.

Plan of Correction: The parent will be asked for the missing information.

Standard #: 22VAC40-185-330-B
Description: Based on observations, it was determined that there is not enough mulch in many of the fall zones on the playground. Evidence: There is not enough mulch in the following playground areas: beneath the preschool swings, beneath the red dome climber, beneath the climbing fire pole, at the bottom of each of the three slides, and beneath the tire climber. In each of these areas, the resilient surfacing has worn down to a very thin layer of mulch and/or to the layer of dirt beneath the mulch.

Plan of Correction: We just installed new mulch but the playground was used by the community and now much of the mulch has been moved. Additional mulch will be ordered and installed.
