Discussed development assessment and transitions.

A complaint investigation has been completed at the center. Records were reviewed, class room equipment was observed, and staff interviews were conducted in response to a complaint alleging a forbidden action had occurred and ratios were in question. Based on the preponderance of evidence gathered, only the ratio allegation was found to be out of compliance. During the course of the investigation, it was also found that background checks were not current for all staff. Thank you for your cooperation in this manner.

Description: Based on observation, record review, and staff interviews, the Toddler room was over ratio during nap when a child under 16 months was sleeping in the room with the 8 children and only one staff present in the class. The infant room was over ratio this morning when there were 5 infants with only 1 teacher for approximately 15-19 minutes. Evidence: 1. Staff stated they double at nap so having 8 with 1 was okay but when the Inspector asked if all were 16 months or older, the staff stated there was one child who was just 15 months. 2.The infant head count sheet showed that there were 3 infants and 2 toddlers in the Infant room with only 1 staff at around 730 in the morning on April 18. The staff who was scheduled in the room confirmed she was alone with 5 children.

Plan of Correction: First, we though we only needed a letter from the parent that it is okay to move a child up but will for now on have a written assessment completed by the program lead to ensure we stay within ratio. We have been hiring people because a staff member abruptly left last week. In the meantime, we will continue to monitor numbers.

Description: Based on record review, the center has not renewed background checks for 2 staff before three years since the dates of the last background checks. Evidence: 1. The Inspector reviewed 4 staff files on March 11, 2016. 2. Two staff files had search of central registry that had expired- 1 expired in April, 2015 and another on March 5, 2016.

Plan of Correction: This was corrected and completed clearances are maintained in their files.
