Today's visit was a monitoring inspection. The inspection began at 11:30am and concluded at 4:00pm. There were a total of fifty-five children in care with eight direct care staff. During the inspection all six classrooms were viewed, lunch time was observed, nap time was observed, four out twelve staff records , four out of seventy-one children's records were reviewed, arrival of school age children was observed, one out of three vans was boarded and viewed (Miniwld5), the playground was viewed, three medications were viewed along with authorizations, and some policies and procedures were reviewed. Standard #: 22VAC40-185-40-G Description: Based on the inspector's review of a sample of four out of seventy-one children's records, the center failed to ensure a record included documentation of viewing a child's proof of identity. Evidence: The record for child #2 does not include documentation of viewing the child's proof of identity. Plan of Correction: The PD has already notified the parent regarding the proof of identity.
Standard #: 22VAC40-185-500-A Description: Based on the inspector's observations in the toddler classroom, the center failed to ensure hand washing took place after a diaper change. Evidence: The inspector observed three back to back diaper changes and the staff member failed to wash her hands in between each diaper change. Plan of Correction: The PD will review diapering procedures with all staff involved with diaper changing.
Standard #: 22VAC40-185-500-B Description: Based on the inspector's observations in the toddler classroom, the center failed to ensure the diapering area was equipped with required supplies. Evidence: The diaper changing area in the toddler classroom was not equipped with single use exam/surgical gloves as required by the standard. Plan of Correction: The diapering area will be equipped with gloves by the end of the program day. The PD will instruct staff to request additional gloves before the last pair of gloves are used.
