Comments:
An unannounced renewal inspection was conducted today by the licensing inspection. The center director was present and assisted during the inspection. All required information was posted and a sample of staff and children's records were reviewed on site. Medications, the playground and the bus were inspected today. The door to the kitchen was locked and previous violations had been corrected.

Observations were done in all classrooms.
4's 2:26 The children were resting. Their art work was displayed throughout the room.
3's 2:19 This group was also resting. They had completed a finger paint version of the earth.
2's 2:22 The children were napping and their art projects were displayed in the classroom.
toddlers 1:10 This group was sleeping. There are new shelves and a magnetic board in this room.
young toddlers 1:8 The children were sleeping. There are new shelves and new tables and chairs in this classroom.
infants 3:12 Most of the children were awake and either in an infant seat or on the floor. One child was walking around in the room.
schoolagers 1:5 This group arrived as the inspector was leaving.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not maintain the preschool bathroom in a safe condition. Evidence: One of the child size toilets in the bathroom between the 2 and 3 year old classrooms has a tank top with a broken corner and a sharp edge.

Plan of Correction: I will email my supervisor who will put in a request for a plumber to be sent out. I will cover the broken area until the top is replaced.

Standard #: 22VAC40-185-340-F
Description: Based on observation, the facility did not ensure sight and sound supervision during nap time. Evidence: During rest time, the 3 yr old children nap on both levels of the classroom. There were 20 children in the classroom with one staff, adhering to doubling the nap time ratio. One staff cannot provide sight supervision to all the children while resting on both levels of the classroom during nap time (due to the half wall).

Plan of Correction: I will get with staff to work out room arrangement that will accomodate all the cots. If that doesnt work we will have 2 staff present.
