22VAC40-191 Background Checks (22VAC40-191)

An unannounced morning inspection was completed today. The Inspector observed classes, reviewed ratios, and observed groups. The Inspector also reviewed safety procedures, special care provisions, medication, staff records, and attendance lists. Children in the three's class had a learning activity going on and the preschool children were observed in an activity. The Inspector discussed observations related to the two's class and physical plant. Please call with questions 703-479-4679

Standard #: 22VAC40-185-270-A
Description: Based on observation and staff interviews, areas and equipment inside of the center, have not been maintained in a clean, safe and operable condition. Evidence: 1. Floors throughout the center had dirt and sticky residue around the baseboards in several classrooms. The stripe carpet in the school age class had tears around the edges and the bathroom floors were dirty. 2. The hand washing area in the two's, three's and school age classes were not clean and the walls and cabinets in these areas were also dirty and stained. 3. Some of the Baseboards, floors and doors had stains and sticky residue on the surface and the chairs in the two's class had stains and dirt. 4. Some walls had peeling paint and/ or ripped pictures. 5. The Two's bathroom had 4 card board boxes stacked and a hula hoop was next to a toilet. 6 Staff were interviewed about the cleanliness and stated the cleaning crew did not clean over the weekend.

Plan of Correction: Center is addressing issues with cleaning vendor. Staff have been retrained on as you go cleaning. A deep scrub and carpet cleaning is scheduled for early May. Cleaning by vendor will be 3 x week. Rugs, boxes and hola hoops were discarded in trash bins. Teachers will be reminded about appropriate storage locations.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the toddler's class, the three's class, and the two's class did not have protective covers over all of the electrical outlets.

Plan of Correction: Uncovered outlets corrected on 4/25/2016. Staff instructed to check covers daily.

Standard #: 22VAC40-185-500-A
Description: Based on observation of diaper changes in the two's class, staff did not wash their hands with soap and running water before and after helping a child use the toilet or a diaper change. Evidence: 1. Staff were observed changing two diapers in the class. During the observation, the staff did not wash her hands or the child's hands after completing each diaper change. A total of two consecutive diaper changes were observed and neither included hand washing before or after each diaper change.

Plan of Correction: Staff have been counseled and trained on appropriate diaper changing procedures. Assistant director for infants, toddlers and twos has been assigned to classroom to ensure procedures are followed.

Standard #: 22VAC40-185-550-B
Description: Based on record review, the Emergency Prepardenss plan was not complete because the manual did not include details specific to the center including: 1.Name and phone numbers for Emergency Officers. 2. Location in building for shelter-in-place procedures. 3. Evacuation route and address for relocation.

Plan of Correction: Emergency management plan has been updated and all required information has been included.
