An unannounced renewal inspection was conducted today, April 27, 2016 to determine the center's compliance with licensing standards. The inspector arrived at 10:20 am and concluded the inspection at 1:00 pm. Upon the inspectors arrival, children were observed in a variety of activities. Children in lower elementary were working in their work cycles, some children were diagraming the parts of a flower, having snack and working one on one with staff preparing for the science fair that will be held on Friday. Upper elementary children have been studying Japan and had models they had made on display. Children were observed reading about mind craft/ building games, having lunch and drawing. Upstairs the two primary classrooms were observed returning from outside play. Children washed their hands and prepared for lunch. Families provide their own meals. Children also worked on concepts of sink/ float and maps. Later children will be creating healthy smoothies for a snack. Children were observed outside on the playground. Children played with balls, hula hoops and in the club house structure. A garden has been planted and children help to tend to the plant. Staff interacted in a positive manner with children as they circulated around the room. The facility has age and stage appropriate materials and supplies. The facility had the following posted: License, last inspection, emergency numbers, evacuation maps/procedures, daily schedule and various parent information. The first aid kit and non-medical emergency supplies were inspected and found complete on both floors of the facility. The playground was inspected. The following ratios (staff to children ) were observed in each classroom: Lower elementary: (6-8 years): 24 children with 2 staff. Upper elementary: (9-12 years) 14 children with 2 staff. Primary I: (3-6 years) 14 children with 2 staff. Primary II: (3-6 years) 13 children with 2 staff while napping. The last evacuation drill was conducted on 03/07/2016. Two shelter-in-place drills were documented on 06/09/2015 and 11/16/2015. A shelter-in-place drill for 2016 year was conducted on 03/07/2016. The Emergency Preparedness Plan was reviewed on May 9, 2015. The facility has current liability Insurance. Health Inspection: 01/11/2016 Fire Inspection: 11/12/2015 There were no medications on site. One staff has current EMAT certification. Staff have current CPR & First aide training along with Daily Health Observation training. The violations from the previous inspection on December 16, 2015 were checked for corrections. There were no repeat violation during today's inspection. Description: Based on review of staff records, the facility did not obtain a sworn statement before three years since the date of the last sworn statement. Evidence: The most recent sworn statement for staff #1 was dated 12/10/2012, exceeding the three year allowance.

Plan of Correction: Per the Director: I will have the staff member update the sworn statement for the record. We will review staff record information to insure information is updated in a timely manner.
