22VAC40-191 Background Checks (22VAC40-191)
Thank you for your assistance during this unannounced inspection. This on-site visit was conducted on April 27, 2016 from 10:40 a.m. through 2:00 p.m. On this date, there were seventy-one (71) children in the care of eighteen (18) staff members in nine classrooms. There are a total of seventy-nine (79) children enrolled. Five (5) children's records and twenty-one (21) staff records were reviewed and updated. The medication and medication paperwork was reviewed for one child. Areas of non-compliance may be found on the violation notice. Risk scores were discussed. Areas observed were outside play, free choice activity, educational centers, indoor movement activity, hand washing/bathroom and diapering procedures, infant feeding, lunch and nap. All interactions between staff and the children were appropriate and the children seemed comfortable with the daily routine. There are a wide variety of age/stage materials for the children in care. Standard #: 22VAC40-185-160-A Description: Staff member T did not have documentation of obtaining a tuberculosis screening within 21 days of employment. Evidence: Based on record review and conversation with the director, it was confirmed Staff T did not have documentation of obtaining a tuberculosis screening. Plan of Correction: Staff T will provide documentation of obtaining a tuberculosis screening.
Standard #: 22VAC40-185-280-B Description: One bottle of Ecolab sanitizing spray was located on a ledge near the bathroom area in Classroom # 1. Evidence: Based on observation and discussion with the teacher it was confirmed the spray bottle of Ecolab sanitizer was not in locked area. Plan of Correction: The staff placed the Ecolab sanitizing spray in an area inaccessible to the children in care.
Standard #: 22VAC40-185-520-C Description: One jar of coconut oil was not labeled with the child's name in Classroom # 4. Evidence: Based on observation and discussion with the teacher, it was confirmed the coconut oil was not labeled with the child's name. Plan of Correction: The teacher will label the coconut oil with the child's name. In the future, diaper ointments or creams brought from home will be labeled with the child's name.
Standard #: 22VAC40-185-560-G Description: One infant bottle and one jar of Earth's Best Butternut Squash were not labeled with the child's name and dated in Classroom # 4. Evidence: Based on observation and discussion with the classroom teacher, it was confirmed one infant bottle and one jar of butternut squash were not labeled with the child's name and dated. Plan of Correction: All food and bottles brought from home will be labeled with the child's name and dated.
Standard #: 22VAC40-191-40-D-1-B Description: Staff member N, P, and Q did not have documentation of obtaining a central registry search. Evidence: Based on record review and conversation with the director, it was confirmed Staff N, P, and Q did not have documentation of obtaining a central registry search. Staff N, date of hire was 11/16/15. Staff P, date of hire was 11/10/15. Staff Q, date of hire was 10/15/14. Plan of Correction: Central registry searches will be completed for Staff N, P, and Q. In the future, all central registry searches will be completed within 30 days of employment.
