22VAC40-191 Background Checks (22VAC40-191)

Technical Assistance:
Discussed ways to incorporate shady areas on the playground on days that the licensed programs are open during the months of June, July, and August.

An unannounced full renewal inspection was conducted today. There were 58 children in care, with a total of 8 staff members assigned to their direct supervision. The children were observed making letter books, listening to stories, attending indoor recess class, eating snack from home, and listening to group interactive electronic books. The playground has recently been replaced with a much larger variety of activities for the children. This inspection was completed between approximately 9:30 am and 11:30 am. Standard #: 22VAC40-185-40-K
Description: Based on a review of administrative documents, it was determined that the playground plan is not complete. Evidence: The playground safety plan does not include a statement indicating staff responsibility for maintaining the resilient surfacing.

Plan of Correction: We recently renovated the playground and installed poured-in-place rubber surfacing. I will add a statement that staff are to report any damage to the rubber surfacing to the preschool director.

Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: Child record #2 and #7 do not include each father's work phone number.

Plan of Correction: Both parents will be asked for the missing information.

Standard #: 22VAC40-185-280-G
Description: Based on observations, it was determined that one spray bottle of a potentially hazardous substance was not labeled to indicate the contents. Evidence: In Room 2, one spray bottle of table cleaning solution was not labeled to indicate the contents.

Plan of Correction: The bottle will be labeled today.

Standard #: 22VAC40-185-510-A
Description: Based on a review of children's medications, it was determined that two children's medications were on site without completed medication authorization forms. Evidence: Two children's prescription emergency medications were on site without completed medication authorization forms. There was an allergy action plan for each child's medication, but there was not a medication authorization form for each child.

Plan of Correction: The parents will be asked for medication authorization forms.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children's medications, it was determined that one child's required medication was not on site. Evidence: According to one child's allergy action plan, the doctor has indicated the need for an over-the-counter allergy medication to be administered to the child along with a prescription medication in the event of an allergy. The child's prescription medication was on site, but the parent had not provided the over-the-counter medication for the child.

Plan of Correction: We have a school supply of the over-the-counter medication that is listed on the action plan and authorization form. The parent will be asked to bring the medication for their child if it is still needed.
