22VAC40-191 Background Checks (22VAC40-191)

An unannounced renewal inspeciton was done today May 3, 2016 beginning at 9:10 AM and ending at 4:00 PM. There were 187 children in care today. The children were observed during their free play in centers which included housekeeping, writing, blocks, table activities, macaroni table and learning about letters with stamps. The children were also having stories read to them as well as discussing healthy/unhealthy food choices. Lunch, afternoon snack and nap were also observed. Five medications were reviewed as well as the outside play area. The inspector also reviewed a sample of five files of the enrolled children which included some newly enrolled children. A sample of nine staff files were also reviewed and this review included newly hired staff. The violations were discussed with the administrative assistant today as well as corrective action. Description: Based on observation the center did not obtain the required documentation for staff TB test. Evidence: 1. During the inspection the LI reviewed 9 staff files and staff # 2 & 4 did not have a physicians signature on the most recent TB test verification.

Plan of Correction: The administrative assistant indicated these TB tests will be given to staff to obtain a physician's signature.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not keep a portion of the outside play area in good repair. Evidence: 1. During the inspection the LI observed on the upper outside fenced play area that a portion of the lattice had come loose or broken from the underpinning of the storage building. This building is located inside the fenced play area.

Plan of Correction: The administrative assistant indicated she would have maintenance repair the latttice.
