An unannounced monitoring inspection was conducted beginning at 4:05 p.m. to 5:45 p.m. with the director and one staff. There were twenty-two children in care ranging in age from five years to twelve years old with two staff. Children were observed during outside play. After outside play, children were observed during transition inside. Children prepared for snack which was rice crispy treats and milk. After snack, children played at their choice of centers which included legos and board games. After centers, children played a basketball game. Standard #: 22VAC40-185-60-A
Description: Based on review of documentation, children's records were not complete. Evidence: Child #2, did not have a full address for a second emergency contact. Child #3 did not have a second emergency contact. Child #4 did not have an address for a second emergency contact.

Plan of Correction: This information has been requested from the parents. The information will be requested again.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all equipment was maintained in safe, operable condition. Evidence: A blue and green striped rug has a torn and frayed edge. A red rug has a torn and frayed edge. The large plaid rug has a torn, worn corner.

Plan of Correction: The torn rugs were removed during the inspection.

Standard #: 22VAC40-185-560-F
Description: Based on observation, the weekly snack menu was not up to date. Evidence: The snack menu that was posted was dated 5/2-5/6/2016.

Plan of Correction: The current weekly menu was posted during the inspection.
