Comments:
Today an unannounced monitoring inspection was conducted beginning at 12:15pm. There were 64 children present with 10 staff and the Director. Children were observed washing hands, eating lunch and napping. Three medications with authorizations were reviewed today in addition to 5 children's records and 1 new staff record. This inspection, including the exit interview, ended at 2:30pm. Should you have any questions, please call.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on direct observation, two cots in the 3's classroom are not in safe condition. Evidence: Two cots with mesh bottoms are fraying with threads coming loose at the ends. The corner threads are frayed to the point of potentially breaking if a child's weight is shifted while using the cot.

Plan of Correction: We ordered replacement pieces for the cots but they were the wrong parts. We will send those back and replace the cots or the mesh part of the cots. We will not use these cots until they are replaced.

Standard #: 22VAC40-185-400-B
Description: Based on direct observation, guidance was not observed as constructive and appropriate when redirecting a child. Evidence: The pre-k 1 class was going down two flights of stairs back to their classroom. One child refused to put shoes back on and was not cooperating with the teachers or following directions. When a teacher (#1) tried to guide the child down the steps with both hands underneath the child's arms, the child fell limp to get away from the teacher. The teacher then picked the child up by their arms, pulled the child off the ground, and carried the child down the bottom flight of steps by the child's arms outstretched above their head with their feet off the ground (stairs).

Plan of Correction: We will continue to provide behavioral guidance training, continue to provide support to our staff, and encourage our staff to bring children to the office with the Director if a child needs to de-escalate. Today we were unable to bring the child to the office to de-escalate due to staffing shortages. We will also add walkie-talkies to our program so staff can call for help should a situation arise that warrants another staff person.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medications and authorizations, 3 medications for children are expired or their medication authorization is expired. Evidence: There is a prescription medication for child #1 that expired 4/16. There are two medications for child #2 that expired 2/16 (prescriptions) and 3/16 (OTC). A physician's authorization for child #2 for both medications (an OTC medication and a prescription medication) expired 2/2016. No medications were returned to the parents or disposed of within 14 days.

Plan of Correction: We will send these home today and if the parents wish to continue with as-needed medications on-site, we will request that they bring in current medications and updated authorizations.
