Comments:
The licensing inspector conducted an unannounced monitoring inspection Wednesday, May 18, 2016 from 11:35am to 2:13pm. The children were observed finishing lunch and preparing for rest time. The infants were observed being fed and playing in an exersaucer. Interviews were held with staff throughout the inspection. Additionally, when appropriate the inspector interacted with children in each classroom. All classrooms and playgrounds were inspected today as well as the facility?s vans. Three children?s records and one employee record were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 4/19/2016 Last shelter-in-place drill: 3/22/2016 Last fire inspection: 6/4/2015 Last health inspection: 4/4/2016 Total enrollment: 49 Today, the following child to staff ratios were observed: Infants ?4:1 Toddlers ?5:2 Twos ? 11:2 Two and Half-8:2 Threes ?10:1 Pre-K ?10:1 (and one intern) School Age ?not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-420-E-3
Description: Based on a review of children's records and staff interview on 05/18/2016, the center did not ensure that two of the three records reviewed contained documentation of the annual updates. EVIDENCE: 1. The most recent update documented in the record of Child #1is dated 6/5/2014. 2. The most recent update documented in the record of Child #2 is dated 4/22/2015. Staff #2 stated that updates were completed in March 2016, however documentation was not kept. Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Such sharing of information shall be documented.

Plan of Correction: Updates are done September and January. Will have a form included in that packet to document updates

Standard #: 22VAC40-185-450-A
Description: Based on observation on 05/18/2016, the center did not ensure that lines on the cots that the children were resting on consisted of a top and bottom cover. EVIDENCE: Several children in the Pre-K and 2 1/2 room did not have a top and bottom cover on their cot during nap time. Cots, and mats used by children other than infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Plan of Correction: Corrected on site. Extra covers at the center were used to provide a bottom cover for the cots that did not have one.
