Comments:
An unannounced monitoring inspection was conducted today from approximately 2:30 to 4:00pm. Ratios for all four groups were found in compliance. The older children were playing outside and the younger children were getting up from nap. They were also eating snack, listening to stories, singing songs and some were making gifts for Mother's Day. New outdoor play equipment has been added and a new fence has been installed on the playground. Medication procedures were reviewed, and staff with current MAT, first aid and CPR training were on site. If you have any questions, I can be reached at 703-479-4709.

Standard #: 22VAC40-185-40-D
Description: Based on observation, the license was not posted in a place conspicuous to the public. Findings: At the time of the inspection, the inspector observed the license posted in an inconspicuous area of the office that is located on the upper level of the building. The information for parents is posted on the main level of the building where all the classrooms are located.

Plan of Correction: We will relocate to front of building.

Standard #: 22VAC40-185-70-A
Description: Based on record review, there was no documentation that a new staff person had obtained the required orientation training. Findings: At the time of the inspection, the inspector found that the staff person had not signed the orientation training form indicating that she had obtained all the training.

Plan of Correction: The staff person signed the orientation training form today.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the inspector found that there was no toilet seat on the toilet in the toddler bathroom.

Plan of Correction: Seat has been ordered, but has been on back order. Will follow up with vendor.

Standard #: 22VAC40-185-510-E
Description: Based on observation, the inspector found that a container of medication for one child was not labeled with the child's name, dosage amount, and time to be given.

Plan of Correction: The medication was labeled at the time of the inspection.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on record review, there was no documentation of a CPS search of the central registry for a new staff person who started on February 16, 2016. Documentation indicated the search was requested on March 8, 2016, three weeks after the employee started working.

Plan of Correction: Search will be sent, receipt filed to make sure all is up to date.
