Technical Assistance:
Technical assistance was given regarding the clear documentation of the items served for weekly snack when the center chooses to provide the food.

Comments:
An unannounced monitoring inspection was conducted on June 7, 2016. At the time of entrance there were 21children and 3 staff members present. The sample of records reviewed consisted of 3 children's records and the emergency contact information and health related information for 3 staff. Children were observed preparing for and eating sanck and playing large group games in the gym. The inspection began at approximately 3:45 p.m. and ended at approximately 5:25 p.m.. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed with the center director during the exit interview.

Standard #: 22VAC40-185-40-I
Description: Based on documentation review and staff interviews, the licensee did not ensure the center developed an anual written procedures for injury prevention. Evidence: During the inspection on June 7, 2016, the most recent copy of the injury prevention plan available for review was dated school year 2010/2011.

Plan of Correction: The center director will update the plan.

Standard #: 22VAC40-185-550-M
Description: Based on a documentation review and staff interviews, the licensee did not ensure the center maintained a written record of children's serious and minor injuries in which entries are made the day of occurrence and included all required information. Evidence: During the inspection on June 7, 2016, three injury reports were reviewed and one did not include the required 2 signature of either the parent of the injured child and a staff member or two staff members.

Plan of Correction: The center director will make sure there are two signatures

Standard #: 22VAC40-185-560-K
Description: Based on observation and staff interviews, the licensee did not ensure tables used for feeding were sanitized before and after each use for feeding. Evidence: During the inspection on June 7, 2016, 8 children were observed eating snack and the table was not sanitized before they ate. The definition of "sanitized" means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution (i.e., bleach solution or commercial chemical disinfectant) or physical agent (e.g., heat). The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution.

Plan of Correction: The center director will have a meeting with staff and make sure they are aware of that and do it correctly in the future.
