22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today by the licensing inspector. Some but not all standards were checked today and a sample of staff and children's records were reviewed on site. All required information was posted and observations were done in all classrooms in use by the children in care today. staff/child ratios observed: toddlers -1:10 Children were napping 2/3's 1:12 Children were napping (observed also on playground) 4's 2:8 The children were preparing for lunch. 4' s pre K 2:8 This group was outside than came inside and prepared for lunch. schoolage 1:9 Schoolage children arrived afterschool later during the inspection. infants 3:8 All the children were awake and playing on the floor with a variety of materials and interacting with staff. lunch today - hot dogs, beans, pineapple, bread milk afternoon snack - animal crackers and milk health inspection 10/25/15 fire inspection 9/16/15 evacuation drill 6/3/16 shelter in place drills for 2015 7/3/15 & 12/5/15 (two required for 2016) daily health observation - 2 staff with current training emergency preparedness plan reviewed 5/16 MAT (medication) training - one staff approximate time on site: 11:30 am - 3:30 pm
Standard #: 22VAC40-185-130-A Description: Based on review of a sample of children's records, the facility did not ensure immunizations were obtained before a child can attend the center. Evidence: Child #4 start date 12/4/15 Documentation of immunizations was dated 4/22/2016. The documentation was not dated within the required time frame. Plan of Correction: We will obtain documentation of immunizations within the required time.
Standard #: 22VAC40-185-140-A Description: Based on review of a sample of children's records, the facility did not ensure documentation of a physical examination was obtained within 30 days after beginning attendance the center. Evidence: Child #4 start date 12/4/15 There was no documentation of a physical examination on file for this child. Plan of Correction: We will request information from the parent.
Standard #: 22VAC40-185-270-A Description: Based on observation, the facility did not maintain areas outside in a safe condition. Evidence: There are three landscape timbers on the playground that have areas that are splintering with sharp edges protruding up such that there are a potential hazard for children. The timbers are located on the side of the schoolage playground and on two sides of the toddler playground near the storage shed. Plan of Correction: We will remove or repair the timbers .
Standard #: 22VAC40-185-350-D Description: Based on observation, the facility did not ensure required staff/child ratios during rest period. Evidence: In the toddler room, age 16-24 mos., there were 10 children resting with one staff supervising. The staff/child ratio for this age group is 1:5, and may be doubled at rest time. Ratios are determined by the age of the youngest child in the group. An infant (child under 16 months) was In the toddler room today during rest time which changes the ratio for the group to 1:4. The infant ratio may not be doubled at rest time, therefore it remains at 1:4 at all times. Plan of Correction: We will move the child back to the infant room.
Standard #: 22VAC40-185-430-B Description: Based on observation, the facility did not ensure an adequate supply of materials for toddler/preschool children. Evidence: Considering the number of children in each toddler/preschool classroom, additional materials are needed, such as but not limited to items for: stacking & sorting, books, puzzles, science & music items, manipulatives, trucks, cars, housekeeping items, dress up, farm animals etc. Plan of Correction: We will do an inventory of materials on site and add other items needed. We will also work on how to better use existing materials on site.
