22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today by the licensing inspector. The center director was present and assisted with the inspection. Some but not all standards were checked today and all required items were posted. A sample of staff and children's records were reviewed on site today. There was no medication on site for children. Observations were done in all classrooms and first aid supplies were checked.

staff/child ratios observed:
5/6's 1:12 The children were playing with dolls, various games, dinosaurs, lincoln logs and cars. Some were also coloring.
3/4's 1:9 The children were in two groups working on handwriting.
4/5's 1:10 The group was doing circle time followed by coloring a summer picture.
2/3's 1:7 Children were reviewing color and playing in the home living area.
toddlers 1:3 All the children were on the floor playing with trucks, musical toys, puzzles and looking at books.
infants 2:7 Three children were sitting at the feeding/activity table. One baby was almost asleep in an infant seat, while the other three children were on the floor playing with a variety of toys.
schoolagers: 1:16 Several children were playing their own electronics, while others were playing cards, working on hair with their dolls, playing board games and other games.

Description: Based upon observation, the facility did not ensure the temperature of an inside area occupied by children did not exceed 80 degrees. Evidence: The thermostat in the large schoolage classroom indicated the temperature to be 83 degrees.

Plan of Correction: A fan will be used and we will check the cooling system in that room.

Standard #: 22VAC40-185-330-A
Description: Based upon inspection of the playground, the facility did not ensure the playground was free of hazards. Evidence: The open sided house with a blue roof on the playground has numerous areas where there is rust. The roof is broken in 2 places where a child could sustain a cut or scrape. The Tic Tac Toe structure has rusty areas in reach of children. There are sharp areas on the end of the wooden seat of the open sided house with the red roof. The large jeep is rusting in numerous places and the steering wheels are not in place and the steering wheels from the small cars are also not in place, both creating a risk of injury to children.

Plan of Correction: The equipment needing repair will be fixed or removed from the playground by June 27th.

Description: Based upon inspection of the playground, the facility did not ensure sufficient resilient material in the fall zones. Evidence: There is approximately 2 inches of resilient material in the fall zones of the big jeep.

Plan of Correction: We plan to remove the big jeep and may replace it with the small jeep. If so, we will add the necessary depth of resilient material. Children will not play on the equipment until corrections are complete.
