This was an initial inspection for licensure. During this inspection standards related to care of children, programming and record keeping were discussed but not observed. All areas of standards will be observed at the first unannounced inspection. All inspections will be unannounced from now on. The first unannounced inspection will be conducted within 30 - 60 days of receipt of the paper license. Today we discussed two references staff and use/fall zone for playground equipment, risk rating, program leader qualifications, required posting of license and inspection results, and criminal history background checks.
An unannounced initial inspection was initiated on 6/23/16 and concluded on 6/28/16. There were not any children in care during the inspection. The inside and outside of the child care facility were reviewed. Violations observed on 6/23/16 related to the following standards include physical plant, resilient surface, required training and posting of shelter-in-place map.
Standard #: 22VAC40-185-40-K Description: Based on observation, documentation review and interview the center failed to create written playground safety procedures that include a resilient surface maintenance plan. Evidence: 1 - The outside playground includes a wooded climb, slide and swing set on resilient surface. However the center does not have a written resilient surface maintenance plan. 2 - The owner confirms that there is not a written plan as a part of the center procedures as required. Plan of Correction: The center owner will either remove the equipment that requires a written or plan or a written plan will be created he equipment will be removed or the equipment will be inaccessible to children in care.
Standard #: 22VAC40-185-240-D-5 Description: Based on record review and interview the center failed to ensure that there is always one staff member on duty who has obtained daily health observation. Evidence: 1 - Record review showed no staff with daily health observation training. 2 - The owner confirmed that daily health observation training has not been obtained. Plan of Correction: The owner will obtain daily health observation training and ensure that there is someone present at all times when children are in care.
Standard #: 22VAC40-185-270-A Description: Based on observation and interview the center failed to ensure that areas are maintained in safe condition. Evidence: 1 - There is a Christmas tree in a large plastic bag stored next to a water heater in such a way that the bag and tree are touching the water heater. 2 - The owner confirmed that the Christmas tress is stored in such a way that the tree and bag are touching the water heater. Plan of Correction: The tree removed so that it is not near or touching the water heater
Standard #: 22VAC40-185-330-B Description: Based on observation and interview the center failed to ensure that the required fall/use zones are present under and around playground equipment. Evidence: There is a wooden piece of climb, slide and swing equipment that does not have required fall/use zones in front, behind and to the sides of the equipment. 1 - The slide is 6 and 1/2 feet tall. However, there is not 10 12 feet of clear resilient surface (use zone) at the bottom of the slide as required. 2 - The climb, slide and swing equipment does not have the required six feet of use zone to the sides of the equipment. 3 - The swing has less than 14 feet of clear use zone in front and behind the equipment. 4 - The owner confirmed that the climb, slide and swing equipment does not have the required use zones. Plan of Correction: The owner will either create the required use zone around the climb, slide and swing equipment or the equipment will be removed or the equipment will be inaccessible to children in care.
Standard #: 22VAC40-185-550-C Description: Based on observation and interview the center failed to ensure that shelter-in-place maps are posted in a location conspicuous to staff and children. Evidence: 1 - There is not a shelter-in-place map posted in the center 2 - The owner confirms that the shelter-in-place map is not posted in the center. Plan of Correction: The owner will post a shelter-in-place in a conspicuous location as required.
