Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:15am - 12:30pm. There were twenty children in care: four infants with one staff, ten preschoolers with two staff and six school age children with one staff. The children were observed in group activities, dancing and during lunch. Three staff and three children's records were reviewed. Violations from the previous inspection were also reviewed and two were repeated. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center failed to obtain an immunization record. Evidence: 1. The inspector reviewed three children's records. One of three records reviewed (child record #1) did not contain documentation of immunizations. This record was previously reviewed and did contain documentation of immunizations.

Plan of Correction: The parent asked for the immunization documentation to take to the physician's office to record the information on the child's physical form. The director will ask the child's parent to return this documentation for the child's record.

Standard #: 22VAC40-185-140-A
Description: Based on record reviews, the center failed to obtain documentation of a physical examination for a child's record. Evidence: 1. The inspector reviewed three children's records. One of three records reviewed (child record #1) did not contain documentation of a physical examination and child's date of enrollment was 08/24/15.

Plan of Correction: The director documented on 12/22/15 that she requested this information from the child's parent, but to date has not received it. The director will request this information again from the child's parent who is employed on site.

Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed three children's records. One of three records reviewed (child record #1) did not contain the following information: documentation of viewing proof of child's identity and age, addresses for each emergency contact, and allergies were not addressed. Child's date of enrollment was 08/24/15.

Plan of Correction: The director documented on 12/22/15 that she requested this information from the child's parent, but to date has not received it. The director will request this information again from the child's parent who is employed on site.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the outdoor play area in a safe condition. Evidence: 1. The inspector observed the outdoor play area. There was a broken landscape timber with sharp edges surrounding the play equipment. There were roots exposed in the play area creating tripping hazards.

Plan of Correction: Maintenance will be requested for the outdoor play area. The roots will be removed and the landscape timber will be replaced. The children will not play in this area until the repairs are completed.
